Undertaking	वचन पत्र
	I, hereby undertake that, I agree to abide by the rules, regulations and guidelines of NIELIT, Local authorities, State Government and Government of India especially related to COVID-19 as issued from time to time.Undertaking	मैं, एतद द्वारा वचन देता हूँ कि, मैं रा.इ.सू.प्रौ.सं. (NIELIT), स्थानीय प्राधिकारियों, राज्य सरकार और भारत सरकार द्वारा समय-समय पर विशेषतः कोविड-19 के बारे में जारी किए गए नियमों, विनियमों और दिशानिर्देशों का पालन करने के लिए वचनबद्ध हूँ।
	1. I shall maintain social distancing while commuting to and from the examination centre and from the time of entry at examination centre till the exit from the examination centre.	1. मैं परीक्षा केंद्र में आने और वापस जाने के दौरान और परीक्षा केंद्र में प्रवेश के समय से परीक्षा केंद्र से बाहर जाने तक सामाजिक दूरी बनाए रखूंगा।
	2. I shall bring soap/hand-sanitiser (up to 50 ml in transparent bottle), face mask, gloves and water in transparent bottle for personal use.	2. मैं निजी उपयोग के लिए साबुन / हैंड-सैनिटाइजर (पारदर्शी बोतल में 50 मिली तक), फेस मास्क, दस्ताने और पारदर्शी बोतल में पानी लाऊंगा।
	3. I shall wear a face mask and gloves properly at all times and if I fail to do so I shall not be allowed entry into the examination centre /expelled from examination centre at any time	3. मैं हर समय फेस मास्क और दस्ताने ठीक से पहनुंगा और अगर मैं ऐसा करने में विफल रहता हूँ तो मुझे परीक्षा केंद्र में प्रवेश की अनुमति नहीं दी जाएगी / किसी भी समय परीक्षा केंद्र से निष्कासित किया जा सकता है।
	4. I understand that I may need to briefly remove the face mask to confirm my identity or to capture a photo at check-in or any time while I am present at examination centre.	4. मुझे ज्ञात है कि चेक-इन के समय पर या परीक्षा केंद्र में मौजूद रहने के दौरान किसी भी समय पर मुझे अपनी पहचान की पुष्टि कराने के लिए या फोटो को कैप्चर कराने के लिए फेस मास्क को थोड़े समय के लिए हटाने की आवश्यकता हो सकती है।
	5. I agree to undergo thermal scanning before entering examination centre and I understand in case of high temperature, I shall not be permitted to enter the examination centre and take examination.	5. मैं परीक्षा केंद्र में प्रवेश करने से पहले थर्मल स्कैनिंग के लिए सहमत हूँ और मुझे ज्ञात है कि मुझे उच्च तापमान होने पर, मुझे परीक्षा केंद्र में प्रवेश करने और परीक्षा देने की अनुमति नहीं दी जाएगी।
	6. I shall wash my hands/ use hand sanitizer before entering examination hall.	6. मैं परीक्षा हॉल में प्रवेश करने से पहले अपने हाथ धोऊंगा / सेनेटाईजर का उपयोग करूँगा।
	7. I shall maintain social distancing and stay more than 6 feet away from other people at all times in the examination centre	7. मैं परीक्षा केंद्र में हर समय सामाजिक दूरी बनाए रखूंगा और अन्य लोगों से 6 फीट से अधिक दूरी पर रहूंगा।
	8. I understand that I need to mandatorily submit self-declaration at the entry point of examination centre failing which I will not be admitted to the examination centre. I understand to truly disclose to the best of my knowledge and belief the self-declaration at the entry of the examination centre.	8. मुझे ज्ञात है कि मुझे परीक्षा केंद्र के प्रवेश द्वार पर अनिवार्य रूप से स्व-घोषणा प्रस्तुत करने की आवश्यकता है, ऐसा न करने पर मुझे परीक्षा केंद्र में प्रवेश नहीं दिया जाएगा। मुझे ज्ञात है कि परीक्षा केंद्र में प्रवेश के समय अपने बारे में जानकारी और विश्वास के अनुसार सही-सही बताना है।
	9. I understand that I shall be permitted to enter the examination centre and take examination only if at the time of entry at the Examination Centre on the date and time of my Examination I fulfil the below mentioned condition(s) but not limited to:	9. मुझे ज्ञात है कि मुझे परीक्षा केंद्र में प्रवेश और परीक्षा में बैठने की अनुमति तभी दी जाएगी जब परीक्षा केंद्र में अपनी परीक्षा के दिन और समय पर प्रवेश के समय मैं निम्नलिखित शर्तों को पूरा करूंगा, जो कि इन तक ही सीमित नहीं हैं:
	a. My Aarogya Setu App status is Low Risk or Safe.	a. मेरा आरोग्य सेतु ऐप का स्टेटस लो रिस्क या सेफ है।
	b. I have not been diagnosed/infected with Coronavirus and/or undergoing treatment for COVID-19	b. मुझे कोविड-19 से पीड़ित नहीं पाया गया / या उसका उपचार नहीं किया जा रहा है।
	c. I have not been in close proximity and/or personal contact with someone with a confirmed diagnosis of COVID-19	c. मैं कोविड-19 की पुष्टि वाले किसी व्यक्ति के निकट और / या उसके व्यक्तिगत संपर्क में नहीं रहा हूँ।
	d. I have not had any flu-like symptoms in the last fourteen days, including fever, chills, cold, runny nose, cough, sore throat, shortness of breath, loss of smell/taste or any other symptom related to COVID-19.	d. मुझे पिछले चौदह दिनों में कोई फ्लू जैसे लक्षण नहीं हुए हैं, जिसमें बुखार, ठंड लगना, सर्दी, जुखाम, खांसी, गले में खराश, सांस की तकलीफ, गंध / स्वाद की हानि या कोविड-19 से संबंधित कोई अन्य लक्षण शामिल हैं।
	e. I have not been under fourteen day's home/institutional quarantine or centralized observations demanded by government and healthcare authorities on the examination day.	e. मैं परीक्षा के दिन सरकार और स्वास्थ्य सेवा अधिकारियों द्वारा अपेक्षित चौदह दिन के घर / संस्थागत क्वारंटाइन या केंद्रीकृत ऑब्जरवेशन के अधीन नहीं हूँ।
	f. I am not staying in containment zone.	f. मैं कन्टेनमेंट जोन में नहीं रह रहा हूँ।
	Declaration
	I, undertake that, I have read and understood the above guidelines and shall abide by the same. I have noted that the Examination Authority has the right to withhold/cancel my examination/candidature in addition to any other action as may be deemed fit in the event of any of the statement(s) made by me in the undertaking above being found incorrect or failure to abide by above guidelines.	मैं, यह वचन देता हूँ कि, मैंने उपरोक्त दिशानिर्देशों को पढ़ा और समझा है और मैं उनका पालन करूँगा। मैंने यह भी नोट कर लिया है कि इस वचनपत्र में दिए गए विवरणों में से कोई भी गलत पाए जाने पर या उपरोक्त दिशानिर्देशों का पालन नहीं करने पर, परीक्षा प्राधिकारियों को कोई भी अन्य उचित कार्रवाई करने के अलावा मेरी परीक्षा / उम्मीदवारी को रोक कर रखने / रद्द करने का अधिकार है।
